CoRIS, the Cohort of the Spanish HIV/AIDS Research Network, is examined to understand unplanned pregnancy prevalence, the contributing factors, and the presence of social and partner support during pregnancy.
In the CoRIS cohort, all women, aged 18 to 50 years, who were enrolled from 2004 to 2019 and were pregnant in 2020, were included. We meticulously constructed a questionnaire, separating the domains of sociodemographic characteristics, tobacco and alcohol consumption, pregnancy and reproductive health, and social and partner support. Information was gathered via telephone interviews, which took place from June until the end of December 2021. Non-contrast computed tomography imaging of patients with ureteral stones frequently reveals the presence of perirenal stranding. Perirenal stranding, potentially originating from tears within the collecting system, has been linked to an elevated risk of infection in prior investigations, necessitating broad-spectrum antibiotic therapy and swift decompression of the upper urinary tract. We posited that these patients are also amenable to non-invasive treatment approaches. Retrospectively, we selected patients with ureterolithiasis and perirenal stranding, comparing diagnostic and treatment characteristics, and outcomes of conservative versus interventional strategies, encompassing ureteral stenting, percutaneous drainage, and direct ureteroscopic stone removal. Using radiological extent as a basis, we graded perirenal stranding, assigning it a classification of mild, moderate, or severe. From a group of 211 patients, 98 were treated using conservative methods. Patients assigned to the interventional arm presented with ureteral stones of greater size, situated more proximally within the ureter, displaying more pronounced perirenal stranding, exhibiting elevated systemic and urinary infection parameters, and higher creatinine readings, necessitating more frequent antibiotic administration. Among the conservatively managed group, spontaneous stone passage occurred in 77% of cases, while delayed intervention was required for the remaining 23%. Four percent of patients in the interventional group and 2% in the conservative group ultimately developed sepsis. Among the patients in both groups, no one developed a perirenal abscess. Despite variations in perirenal stranding grades (mild, moderate, and severe) among patients managed conservatively, there was no difference observed in spontaneous stone passage rates and infectious complication rates.
